Tumor neoantigen-specific T cells are a subset of T lymphocytes that recognize and respond to neoantigens—peptides derived from tumor-specific somatic mutations not present in normal tissues. These neoantigens are highly immunogenic because they arise from mutations unique to cancer cells, making them ideal targets for personalized cancer immunotherapy. These T cells can be either CD8+ (cytotoxic) or CD4+ (helper) subsets. Their presence and activity correlate with favorable clinical outcomes following therapies such as immune checkpoint blockade (ICB) and adoptive cell transfer (ACT).
