Tumor-specific immunoglobulin idiotype on malignant B cells (Idiotype (Id))

Target
Idiotype (Id)
Molecular classification
Other (clonotypic immunoglobulin variable-region epitope on B-cell receptor)
01

Overview

The tumor-specific immunoglobulin idiotype is the unique set of antigenic determinants (idiotopes) located mainly within the complementarity-determining regions of the immunoglobulin variable domains on malignant B cells; because B-cell malignancies are monoclonal, all tumor cells share the same surface-immunoglobulin idiotype, making it a truly tumor-specific antigen that can be targeted by patient-specific anti-idiotype antibodies, protein/DNA vaccines, or dendritic cell–based vaccines, and monitored via serum idiotype levels for disease burden and response

02

Mechanism of action

Antibody binding to clonotypic idiotype on surface immunoglobulin leading to immune-mediated killing; Active vaccination inducing anti-idiotype antibodies and T-cell responses against idiotype-derived idiotopes; Dendritic cell presentation of idiotype to elicit tumor-specific T-cell immunity

03

Biological functions

Immune response (antigenic determinants/idiotopes on B-cell receptor variable regions)Tumor antigen/neoantigen surrogate for immunotherapy
04

Disease associations

Cancer (B-cell malignancies including B-cell lymphoma, chronic lymphocytic leukemia, B-cell acute lymphoblastic leukemia)
05

Safety considerations

Extreme tumor specificity leads to individualized manufacturing complexityPotential induction of regulatory T cells when targeting shared idiotopes (e.g., J-region peptides) that can dampen Id-specific responsesVariable immunogenicity and need for potent adjuvantsClinical logistics and time-to-produce for personalized vaccines
