Type 1 fimbriae are filamentous surface appendages found on many Escherichia coli strains, most notably uropathogenic E. coli (UPEC) (UniProt P08191). They are composed of several subunits, with the FimH protein located at the tip acting as a mannose-binding lectin (PubMed: 31110284). This adhesin allows the bacteria to attach to mannosylated receptors, such as uroplakin Ia, on the surface of bladder epithelial cells (StatPearls: Urinary Tract Infection). This attachment is a critical first step in the pathogenesis of urinary tract infections (UTIs), facilitating colonization and subsequent invasion. Beyond UTIs, Type 1 fimbriae are implicated in the persistence of adherent-invasive E. coli (AIEC) in the gut of patients with Crohn's disease (PubMed: 28803827). Therapeutic strategies focus on small-molecule FimH antagonists, often mannosides, which competitively inhibit bacterial binding to host tissues (ClinicalTrials.gov: NCT03943446). By preventing adhesion, these drugs aim to reduce infection rates and provide an alternative to traditional antibiotics.
