Type 1 fimbriae are filamentous surface organelles found on uropathogenic Escherichia coli (UPEC) that play a critical role in the pathogenesis of urinary tract infections (UTIs). These structures are composed of a major subunit, FimA, and a distal adhesin, FimH, which specifically recognizes and binds to terminal mannose residues on uroplakin receptors of the bladder epithelium. This binding is essential for bacterial colonization, invasion of host cells, and the formation of intracellular bacterial communities that contribute to recurrent infections. Because Type 1 fimbriae are vital for the initial stages of infection, they are a primary target for anti-adhesive therapies, such as mannosides and FimH inhibitors, which aim to prevent bacterial attachment without exerting the selective pressure typical of traditional antibiotics. Research into these fimbriae also focuses on their role in biofilm formation and their potential as vaccine candidates to provide broad protection against UPEC-mediated diseases.
