Overview

Type H blood vessels are a recently characterized subtype of bone microvasculature distinguished by high expression of CD31 (PECAM-1) and Endomucin (EMCN). They are uniquely located in the metaphysis near the growth plate and the endosteum and are surrounded by pre-osteoblasts and osteoprogenitors expressing key transcription factors such as Osterix and Runx2. Type H vessels play a critical role in coupling angiogenesis and osteogenesis, supporting new bone formation through reciprocal molecular signaling between vascular endothelial cells and bone-forming/repairing cells. Abundant in young, healthy bone, their numbers decline with age and bone disease, making them a sensitive biomarker for bone loss and a promising therapeutic target for enhancing bone regeneration and treating osteoporosis. Key regulators include VEGF, PDGF-BB, SLIT3, HIF-1α, Notch signaling, and miRNAs. Targeting Type H vessel formation may enhance bone healing but requires caution due to the fine regulation needed to ensure normal bone homeostasis.
