Overview

Type IV pilin structural subunit (PilA) is the primary protein component of Type IV pili (T4P), which are sophisticated surface appendages found in numerous Gram-negative bacterial pathogens, including Pseudomonas aeruginosa and Acinetobacter baumannii (PMID: 25691586). These pili are essential for bacterial virulence, facilitating twitching motility, adherence to host epithelial cells, and the formation of biofilms on both biological and abiotic surfaces (PMID: 15117903). By mediating the initial stages of colonization and environmental sensing, PilA plays a central role in the pathogenesis of acute and chronic infections, such as those seen in cystic fibrosis and ventilator-associated pneumonia. As a therapeutic target, PilA is being extensively researched for the development of subunit vaccines and monoclonal antibodies aimed at blocking bacterial attachment and enhancing immune clearance (PMID: 15608000). However, the significant genetic diversity and antigenic variation of the PilA protein across different clinical isolates present a major challenge for the creation of broadly effective treatments (PMID: 31085610). Furthermore, the ability of bacteria to utilize alternative adhesion mechanisms can lead to therapeutic bypass and resistance.
