Overview

Unfractionated heparin (UFH) is a naturally occurring, heterogeneous mixture of sulfated glycosaminoglycans used as an anticoagulant. It acts primarily by potentiating antithrombin III’s inhibition of key coagulation enzymes. Its clinical utility lies in its rapid onset but requires careful monitoring due to variability in response and significant risks including bleeding complications and HIT. Its effects can be rapidly reversed if needed.

Mechanism of action

Binds to and activates antithrombin III, which then inhibits thrombin (Factor IIa) and Factor Xa, thereby preventing clot formation.
