Overview

Urinary acid-base balance refers to the kidneys’ role in maintaining systemic pH homeostasis. The renal system regulates acid-base balance by reabsorbing virtually all filtered bicarbonate (a major blood buffer), generating new bicarbonate to replace that lost to acid neutralization, and excreting fixed acids—mainly as titratable acid and ammonium[1][2][3][6][7]. The key processes include activity in different parts of the nephron: reabsorption of bicarbonate in the proximal tubule, secretion of hydrogen ions, production and excretion of ammonium (NH₄⁺), and use of urinary buffers like phosphate and creatinine. This regulated balance is crucial for enzyme function and cellular metabolism. Dysregulation can lead to acid-base disorders, such as metabolic acidosis or alkalosis, with major clinical consequences[2][3][4][6]. The concept itself is a collective physiological function, not a discrete molecular target or receptor.
