Overview

Urinary antiseptics are a class of drugs specifically designed to exert antibacterial activity within the urinary tract, targeting bacteria responsible for urinary tract infections (UTIs). They achieve high concentrations in urine but low levels elsewhere in the body. Common agents include methenamine (which releases formaldehyde in acidic urine) and nitrofurantoin (which disrupts multiple bacterial cell functions). They are primarily used for treating and preventing lower urinary tract infections, and for long-term suppression of bacteriuria.

Mechanism of action

Varies by agent: Methenamine releases formaldehyde in acidic urine, disrupting bacterial proteins and DNA. Nitrofurantoin is converted into reactive intermediates that disrupt multiple bacterial cellular processes. Quinolone derivatives (nalidixic acid, oxolinic acid) inhibit bacterial DNA replication enzymes.
