Overview

The urinary microbiome, or urobiome, refers to the complex community of microorganisms residing in the urinary tract, which was historically considered sterile (PMID: 29473368). This ecosystem plays a critical role in maintaining urogenital health by preventing the colonization of uropathogenic bacteria, such as uropathogenic Escherichia coli (UPEC), through competitive exclusion and immune modulation (PMID: 30116202). Dysbiosis within this environment is strongly linked to urinary tract infections (UTIs), interstitial cystitis, and potentially bladder cancer (PMID: 32662324). Therapeutic strategies targeting this system include traditional antibiotics, which aim to eradicate pathogens but often disrupt beneficial flora, and emerging approaches like probiotics, D-mannose, and phage therapy designed to restore ecological balance (PMID: 31133329). Understanding the interplay between commensal microbes and uropathogens is essential for developing precision treatments that minimize the risk of recurrent infections and antimicrobial resistance.

Mechanism of action

Antibiotics exert bactericidal or bacteriostatic effects by inhibiting cell wall synthesis, protein synthesis, or DNA replication in uropathogens. Probiotics and D-mannose function through competitive inhibition of pathogen adherence to the urothelium and restoration of microbial diversity (PMID: 31133329).
