Overview

The urogenital tract epithelium, primarily referred to as the urothelium, is a specialized stratified tissue that lines the urinary tract from the renal pelvis to the proximal urethra (StatPearls, 2023). Its primary biological role is to provide a robust, impermeable barrier that prevents the passage of water, solutes, and toxic metabolites from the urine back into the underlying tissues and systemic circulation (NCBI, 2022). This tissue is uniquely adapted for distensibility, utilizing specialized apical 'umbrella cells' and a surface layer of uroplakin proteins to maintain integrity during bladder filling (PubMed, 2021). Pathologically, the urothelium is the site of origin for most bladder cancers, known as urothelial carcinomas, and is frequently involved in inflammatory conditions like interstitial cystitis (NIH, 2023). While the epithelium itself is a tissue rather than a single molecular target, it is the focus of intravesical drug delivery systems where agents like BCG or Mitomycin C are used to treat superficial malignancies by direct contact (Journal of Urology, 2022). Furthermore, receptors within the epithelial and subepithelial layers, such as muscarinic and vanilloid receptors, are targeted by drugs to manage overactive bladder and sensory disorders (Pharmacological Reviews, 2021). The integrity of this epithelial barrier is a critical factor in both the pathogenesis of urinary diseases and the efficacy of locally administered pharmacological treatments.
