Overview

Uropathogenic Escherichia coli (UPEC) cell surface and biofilm matrix components are a complex array of proteins and polysaccharides essential for the pathogenesis of urinary tract infections (UTIs). The primary components include type 1 fimbriae (specifically the FimH adhesin), P fimbriae, curli fibers, and an extracellular matrix composed of cellulose and other exopolysaccharides (PMID: 30158195). These structures enable UPEC to adhere to the bladder epithelium (urothelium), invade host cells to form intracellular bacterial communities (IBCs), and establish robust biofilms that resist both host immune clearance and conventional antibiotic therapy (PMID: 29445231). Therapeutic strategies targeting these components focus on anti-adhesion agents, such as mannosides that block FimH, and small molecules that inhibit the assembly of pili or curli fibers (PMID: 31434749). By preventing the initial attachment and subsequent biofilm maturation, these therapies aim to reduce the incidence of recurrent UTIs and overcome the challenges posed by multi-drug resistant UPEC strains.
