The urothelial barrier is a specialized, high-resistance permeability barrier of the urinary bladder that protects underlying tissues from toxic urinary constituents and prevents the adherence and invasion of uropathogens. It is a multi-component system consisting of a luminal glycosaminoglycan (GAG) layer, an apical membrane of umbrella cells containing specialized proteins called uroplakins, and a complex of tight junction proteins that seal the paracellular space. In pathological conditions such as interstitial cystitis (IC) or recurrent urinary tract infections (UTIs), the integrity of this barrier is often compromised, leading to pain, inflammation, and increased susceptibility to infection. Therapeutic interventions often focus on restoring this barrier using GAG replenishers like pentosan polysulfate sodium or intravesical instillations of hyaluronic acid and chondroitin sulfate. Conversely, in the treatment of bladder cancer, the barrier presents a challenge for drug delivery, necessitating strategies to temporarily increase its permeability for intravesical chemotherapy.
