The urothelial bladder surface is the specialized interface between the bladder wall and the urine, composed of the urothelium (transitional epithelium) and a protective glycosaminoglycan (GAG) layer (StatPearls, 2023). Its primary biological function is to serve as a robust barrier against urinary toxins, ions, and pathogens, while also acting as a sensory web that transduces mechanical and chemical signals to the underlying nervous system (PMC, 2009). In diseases such as interstitial cystitis (IC) and bladder pain syndrome (BPS), the GAG layer is often damaged or deficient, leading to increased permeability and chronic inflammation (StatPearls, 2023). Therapeutic targeting of this surface involves GAG-replenishing agents like pentosan polysulfate sodium or intravesical instillations of hyaluronic acid to restore barrier integrity. Furthermore, the urothelial surface is the primary site for intravesical therapies in non-muscle invasive bladder cancer, where agents like Bacillus Calmette-Guerin (BCG) bind to surface proteins to trigger a localized immune response, or chemotherapeutic agents like mitomycin C exert direct cytotoxic effects on superficial malignant cells (PMC, 2007; StatPearls, 2023).
