V-set and transmembrane domain-containing protein 5 (VSTM5) is a member of the immunoglobulin superfamily expressed predominantly in the central nervous system, where it regulates neuronal morphogenesis, dendritic complexity, and synapse formation through its cell adhesion-like properties. VSTM5 is an N-linked glycosylated type I membrane protein that modulates both structural and functional aspects of neuron development. Additionally, VSTM5 has emerged as a novel immune checkpoint, closely related to proteins such as VISTA and TIGIT, with capabilities to suppress T-cell proliferation, induce regulatory T cells, and promote antigen-specific tolerance. Its involvement in neuronal development implicates it in disorders such as major depressive disorder and other neuropsychiatric conditions, while its immune regulatory functions position it as a potential therapeutic target for tolerance-based approaches in treating autoimmune diseases. No drug interactions or clinical biomarker applications have been described to date.
