Overview

Vaginal epithelial pH refers to the acidic microenvironment (typically pH 3.5–4.5) on the vaginal surface and luminal secretions, maintained by a combination of epithelial ion transport activity and metabolites produced by commensal Lactobacillus species. Healthy vaginal pH is primarily sustained by lactic acid produced by lactobacilli and by active proton secretion mediated by epithelial cell ion channels and pumps—most notably the V-type H^+-ATPase, Na^+-H^+ exchanger (NHE), Na^+-HCO₃⁻ cotransporter (NBC), and carbonic anhydrase. Estrogen upregulates proton secretion through these mechanisms, facilitating the acidic environment required for barrier function and microbial balance. Loss of acidity (pH above 4.5) is associated with bacterial vaginosis, increased infection risk, and epithelial inflammation, making pH an important biomarker of vaginal health, but not itself a druggable molecular target.

Mechanism of action

Acidifying agents restore protective acidity. Estrogen enhances proton secretion by vaginal epithelial cells, acidifying the lumen. Probiotics increase lactic acid production by supporting lactobacilli, which acidify the vagina.
