Overview

The VEGF-A–bevacizumab immune complex is a molecular assembly formed when the monoclonal antibody bevacizumab (Avastin) binds to its target, vascular endothelial growth factor A (VEGF-A) [1, 6]. This complex is a central component of anti-angiogenic therapy, as its formation effectively sequesters circulating VEGF-A and prevents it from interacting with its receptors, VEGFR-1 and VEGFR-2 [1, 12]. By blocking these interactions, the complex inhibits the signaling pathways responsible for endothelial cell proliferation, migration, and new blood vessel formation in tumors [2, 3]. Unlike other anti-VEGF agents like aflibercept, bevacizumab can form large, heterogeneous multimeric immune complexes [5, 8]. These multimeric structures are significant because they can deposit in the renal glomeruli, which is a primary mechanism behind bevacizumab-induced proteinuria and renal damage [13]. The clearance of these complexes is a key pharmacokinetic process, often involving the neonatal Fc receptor (FcRn) and other catabolic pathways [10]. Monitoring the levels of these complexes and the remaining free VEGF-A is an area of active research for identifying biomarkers of treatment efficacy and safety [7, 9].
