Overview

Vascular endothelium and smooth muscle components refer to the collective cellular and extracellular structures of the blood vessel wall that regulate circulatory hemodynamics. The endothelium is a thin monolayer of cells that acts as a semi-permeable barrier and produces signaling molecules like nitric oxide to induce relaxation of the underlying smooth muscle. Vascular smooth muscle cells maintain structural integrity and control vessel lumen diameter through contraction and relaxation in response to various neural, humoral, and mechanical stimuli. Together, these components are critical in the pathogenesis of cardiovascular diseases such as hypertension and atherosclerosis. While many drugs are described as acting on these components, they typically target specific molecular entities within them, such as G protein-coupled receptors, ion channels, or enzymes like guanylate cyclase.

Mechanism of action

Drugs targeting these components typically act by modulating signaling pathways that control vasodilation and vasoconstriction, such as the nitric oxide-cGMP pathway, the endothelin system, or ion channel activity in smooth muscle cells.
