The vascular smooth muscle cell (VSMC) lineage comprises the specialized contractile cells found within the tunica media of blood vessels. These cells play a fundamental role in hemodynamics by modulating vascular resistance and blood flow through contraction and relaxation in response to neural, humoral, and mechanical stimuli (PMID: 29120317). A defining characteristic of the VSMC lineage is its phenotypic plasticity; cells can switch from a quiescent, contractile state to a synthetic state characterized by increased proliferation, migration, and extracellular matrix production (PMID: 26103977). This transition is a hallmark of vascular pathologies such as atherosclerosis, restenosis after angioplasty, and pulmonary hypertension (PMID: 30135442). While the lineage itself is a cellular category rather than a single molecular target, it expresses a variety of clinically significant proteins, including L-type calcium channels and angiotensin II receptors, which are the primary targets for antihypertensive and vasodilator therapies (PMID: 28838937).
