The vascular smooth muscle contractile machinery and calcium handling pathways represent the integrated physiological system that regulates blood vessel diameter and peripheral resistance. This system relies on the precise control of intracellular calcium concentrations and the subsequent activation of the actin-myosin cross-bridge cycle. Contraction is typically initiated by calcium influx through L-type voltage-gated calcium channels or release from the sarcoplasmic reticulum, which activates myosin light chain kinase (MLCK) via a calmodulin-dependent mechanism [1][4]. This leads to the phosphorylation of the 20-kDa regulatory myosin light chain (MLC20) and force generation. Relaxation occurs when calcium levels drop and myosin light chain phosphatase (MLCP) dephosphorylates MLC20, a process often promoted by the nitric oxide-cGMP pathway [2][3]. Pathological alterations in these pathways, such as enhanced calcium sensitivity mediated by Rho-kinase (ROCK), contribute significantly to the development of hypertension and vasospastic disorders [3].
