The vasomotor center is a functional collection of neurons located in the medulla oblongata of the brainstem that serves as the primary integrator for cardiovascular homeostasis (StatPearls, 2023). It regulates blood pressure and heart rate by modulating the activity of the sympathetic nervous system in response to inputs from baroreceptors, chemoreceptors, and higher brain centers (NIH, 2022). The center is composed of several sub-regions, most notably the rostral ventrolateral medulla (RVLM), which maintains tonic sympathetic tone, and the nucleus tractus solitarius (NTS), which processes afferent sensory information (Wikipedia, 2024). In conditions such as essential hypertension and heart failure, the vasomotor center often exhibits pathological overactivity, contributing to increased systemic vascular resistance (PubMed, 2021). While not a single molecular target, it is the site of action for centrally acting antihypertensive drugs like clonidine and methyldopa, which stimulate alpha-2 adrenergic receptors to inhibit sympathetic outflow (PubChem, 2024). Consequently, pharmacological modulation of this center is an established strategy for managing refractory hypertension, though it is associated with central nervous system side effects like sedation (StatPearls, 2023).
Activation of alpha-2 adrenergic receptors and imidazoline I1 receptors within the medulla oblongata, which inhibits the tonic excitatory drive to sympathetic preganglionic neurons, thereby reducing peripheral vascular resistance and heart rate (StatPearls, 2023; PubChem, 2024).
