Overview

The venous wall is the multi-layered anatomical structure of veins, consisting of the tunica intima (endothelium), tunica media (smooth muscle), and tunica externa (connective tissue) (StatPearls, Anatomy, Thorax, Veins, 2023). It plays a critical role in the circulatory system by facilitating the return of deoxygenated blood to the heart and acting as a primary reservoir for blood volume (NIH, National Library of Medicine, 2022). In various disease states, such as chronic venous insufficiency (CVI), the venous wall undergoes structural remodeling, including smooth muscle hypertrophy and collagen deposition, which leads to valve incompetence and venous hypertension (PubMed, Journal of Vascular Surgery, 2021). Therapeutic strategies targeting the venous wall typically involve venotonic drugs, such as micronized purified flavonoid fraction (MPFF), which enhance wall tension and reduce inflammatory leukocyte adhesion to the endothelium (PubMed, Drugs & Aging, 2019). While not a single molecular target, the venous wall serves as a complex physiological site for pharmacological intervention in cardiovascular and peripheral vascular medicine.

Mechanism of action

Venotonic agents increase venous tone by prolonging the activity of norepinephrine on the smooth muscle cells within the tunica media and reducing capillary hyperpermeability and inflammatory leukocyte adhesion to the venous endothelium.
