Venous wall tone represents the contractile state of the smooth muscle cells within the venous wall, which is essential for regulating venous capacitance and facilitating blood return to the heart [1]. This tone is primarily maintained through sympathetic nervous system activity, specifically via the activation of alpha-adrenergic receptors by norepinephrine [2]. In conditions such as chronic venous insufficiency (CVI), a pathological decrease in venous wall tone leads to vessel dilation, valvular incompetence, and blood pooling in the lower extremities [3]. Pharmacological agents known as venotonics or phlebotonics, such as diosmin and hesperidin, are used to increase venous wall tone by inhibiting the breakdown of norepinephrine or enhancing calcium sensitivity in smooth muscle [4]. Additionally, venodilators like nitroglycerin can be used to decrease tone in specific clinical settings to reduce cardiac preload [5]. Because venous wall tone is a physiological parameter resulting from the integration of multiple signaling pathways rather than a single protein, it is classified as a phenotypic or physiological target rather than a specific molecular receptor [1, 4].
