Viable metabolically active cells represent a physiological state where cells maintain structural integrity and execute biochemical reactions necessary for survival, such as ATP production and nutrient processing (NIH, 2023). This term does not refer to a single molecular entity like a protein or gene, but rather to the collective functionality of a cell population. In pharmacological research, measuring the presence or activity of these cells is a standard method for assessing drug toxicity, cell proliferation, and the effectiveness of anti-cancer or anti-microbial agents (Promega, 2024). Assays like MTT, XTT, or Alamar Blue specifically quantify metabolic activity as a proxy for cell viability by measuring the reduction of substrates by intracellular enzymes (Nature, 2021). While critical for evaluating therapeutic outcomes, viable metabolically active cells is considered an assay readout or a biological endpoint rather than a discrete therapeutic target. Because it encompasses the entire cellular machinery, it lacks the specificity required for a canonical therapeutic target. Instead, it serves as a broad indicator of biological health or the efficacy of a treatment in vitro and in vivo. Drugs that impact this state usually do so by targeting specific pathways like DNA replication or mitochondrial function.
