Overview

The vomiting center, also known as the medullary emetic center, is a functional region in the medulla oblongata that coordinates the complex reflex of emesis (StatPearls, 2023). It is not a single anatomical structure but rather a collection of interconnected nuclei, including the area postrema, the nucleus tractus solitarius, and the dorsal motor nucleus of the vagus nerve (NIH, 2022). This center integrates emetogenic signals from the chemoreceptor trigger zone (CTZ), the vestibular system, the gastrointestinal tract, and higher cortical centers. Various neurotransmitter receptors are highly concentrated within this circuit, most notably serotonin (5-HT3), substance P (NK1), dopamine (D2), histamine (H1), and acetylcholine (M1), which serve as the primary molecular targets for antiemetic drugs (PubChem, 2024). Medications like ondansetron and aprepitant act by blocking these receptors to prevent or treat nausea and vomiting associated with chemotherapy, surgery, or motion sickness. Because it represents a physiological system rather than a single protein, it is considered a central regulatory hub for autonomic and somatic responses during the emetic process (Wikipedia, 2024).

Mechanism of action

Antagonism of specific neurotransmitter receptors such as 5-HT3, NK1, D2, H1, and M1 located within the medullary nuclei to interrupt the emetic reflex arc (StatPearls, 2023).
