Wart tissue refers to the benign, hyperkeratotic growths of the skin and mucous membranes resulting from infection by various strains of the Human Papillomavirus (HPV). These lesions are characterized by an overgrowth of the epidermal layer, specifically the keratinocytes, which are hijacked by the virus to facilitate its replication and assembly (StatPearls, 2023). While not a single molecular target, wart tissue serves as the site for various therapeutic interventions, including chemical cautery, immunomodulation, and antineoplastic agents. Treatments like salicylic acid work by breaking down the thickened stratum corneum, while others like imiquimod stimulate the host's immune system to recognize and clear the virally infected cells (Mayo Clinic, 2023). In more recalcitrant cases, intralesional injections of bleomycin or topical fluorouracil are used to inhibit DNA synthesis within the rapidly dividing cells of the lesion (PubMed, 2022). Understanding the structure and viral load within wart tissue is critical for developing effective topical and intralesional therapies that target the underlying viral infection while minimizing damage to surrounding healthy tissue.
Therapeutic agents targeting wart tissue primarily function through keratolysis, which involves the chemical dissolution of keratin to shed infected cells (StatPearls, 2023). Other mechanisms include the induction of local cytokines via Toll-like receptor 7 (TLR7) activation to stimulate an immune response against the Human Papillomavirus (HPV), and the inhibition of cellular or viral DNA synthesis to arrest the growth of the lesion (PubMed, 2022).
