Overview

The Wilms tumor protein 1 (WT1)-derived peptide-MHC class II complex is a critical immunological target in oncology. WT1 is a zinc-finger transcription factor that is highly overexpressed in various hematological malignancies and solid tumors, while having limited expression in normal adult tissues (UniProt P19544). Peptides derived from the WT1 protein are processed and presented on the cell surface by Major Histocompatibility Complex (MHC) class II molecules, where they are recognized by the T-cell receptors (TCRs) of CD4+ helper T cells (PubMed: 11133741). This interaction is essential for the orchestration of a comprehensive anti-tumor immune response, as CD4+ T cells provide necessary signals for the activation and persistence of CD8+ cytotoxic T cells and can also exert direct anti-tumor effects (PubMed: 15155834). Therapeutic strategies targeting this complex include peptide vaccines, such as Galinpepimut-S, and adoptive T-cell therapies using TCR-engineered CD4+ T cells (PubMed: 28811301). Monitoring WT1 expression levels and specific HLA class II alleles is vital for patient selection and assessing treatment efficacy. The complex represents a bridge between innate and adaptive immunity, facilitating long-term memory against cancer cells.
