Zymogen granule protein 16 homolog B (ZG16B), also called pancreatic adenocarcinoma up-regulated factor (PAUF), is a secretory lectin-like oncoprotein characterized by a Jacalin-related β-prism fold. It is primarily identified as a growth and metastatic factor in pancreatic ductal adenocarcinoma, and aberrantly expressed in other tumors, such as breast, gastric, and ovarian cancers. ZG16B executes multiple biological roles, including carbohydrate binding, regulation of cell adhesion, apoptosis, angiogenesis, and modulation of immune responses via TLR2/TLR4 and related signaling pathways. It is actively involved in tumor proliferation, migration, and immune evasion, and is currently being explored as a diagnostic/prognostic biomarker and therapeutic target. Therapies under investigation include monoclonal antibodies and FAK inhibitors, with ongoing trials specifically in pancreatic cancer. ZG16B has considerable promise but also complexity, playing both immunostimulatory and immunosuppressive roles in the tumor microenvironment
