This drug is a combination of 5-aminosalicylic acid (5-ASA, also known as mesalamine or mesalazine) and sulphasalazine (sulfasalazine). Sulfasalazine itself is a prodrug composed of 5-aminosalicylic acid linked to sulfapyridine by an azo bond. In the colon, bacterial enzymes cleave this bond to release the active anti-inflammatory component, 5-ASA, which acts locally in the intestinal lumen to reduce inflammation. The primary mechanism involves inhibition of prostaglandin and leukotriene synthesis via cyclooxygenase and lipoxygenase pathways, as well as modulation of immune responses through inhibition of nuclear factor kappa-B signaling. This combination is primarily used for induction and maintenance of remission in mild to moderate ulcerative colitis and can be used adjunctively in Crohn’s disease. It may also be indicated for rheumatoid arthritis and juvenile idiopathic arthritis[2][3][6][7][9].
