Overview

ACVBP + rituximab is an intensified immunochemotherapy regimen combining the polychemotherapy ACVBP (doxorubicin, cyclophosphamide, vindesine, bleomycin, prednisone) with the anti-CD20 monoclonal antibody rituximab, used primarily for diffuse large B-cell lymphoma (DLBCL). In randomized trials, adding rituximab to ACVBP improved event-free and progression-free survival versus ACVBP alone in young patients with localized low-risk DLBCL, and R-ACVBP improved survival compared with standard R-CHOP in patients aged 18–59 with low-intermediate IPI risk, albeit with increased hematologic toxicity. Mechanistically, rituximab mediates B-cell depletion via CD20-dependent cytotoxicity (CDC, ADCC, apoptosis), while ACVBP components provide cytotoxic chemotherapy targeting DNA replication/repair and microtubules.
