Overview

This is a combination of four drugs: adalimumab, azathioprine, certolizumab pegol, and infliximab. Adalimumab, certolizumab pegol, and infliximab are monoclonal antibodies that inhibit tumor necrosis factor alpha (TNF-alpha), a cytokine involved in systemic inflammation. Azathioprine is an immunosuppressive antimetabolite that inhibits purine synthesis and reduces lymphocyte proliferation. These agents are primarily used in the management of moderate to severe inflammatory bowel diseases (IBD) such as Crohn’s disease and ulcerative colitis, as well as other autoimmune conditions like rheumatoid arthritis. Combination therapy with anti-TNF agents plus azathioprine has been shown to be more effective than monotherapy for inducing remission in IBD; however, combining multiple anti-TNF agents together (adalimumab + certolizumab pegol + infliximab) is not standard clinical practice due to overlapping mechanisms and increased risk of adverse effects[1][2][5][8]. The rationale for combining an anti-TNF agent with azathioprine is based on evidence that immunomodulators can reduce antibody formation against biologics and increase drug levels[1][6].
