Overview

This is a combination therapy consisting of three drugs—adalimumab, azathioprine, and metronidazole—used primarily in the management of Crohn’s disease and other inflammatory bowel diseases (IBD). - **Adalimumab** is a monoclonal antibody that targets tumor necrosis factor alpha (TNF-α), reducing inflammation by inhibiting this pro-inflammatory cytokine. - **Azathioprine** is an immunosuppressive antimetabolite that interferes with purine synthesis, thereby suppressing lymphocyte proliferation. - **Metronidazole** is an antibiotic and antiprotozoal agent effective against anaerobic bacteria and certain parasites; it works by generating free radicals that damage microbial DNA. The rationale for combining these agents includes targeting multiple pathogenic pathways in IBD—immune modulation (adalimumab, azathioprine) and infection control or prevention of post-surgical recurrence (metronidazole). However, clinical studies have shown mixed results regarding the superiority of such combinations over monotherapy. For example, adding azathioprine to adalimumab does not significantly improve clinical remission rates compared to adalimumab alone but may enhance early endoscopic response[6][9]. Metronidazole has been used post-operatively to reduce recurrence risk[2][3].
