Overview

This is a nonabsorbable topical combination of three antimicrobial agents—amphotericin B, polymyxin E (also known as colistin), and tobramycin—used primarily for local decontamination of the oropharynx in critically ill or immunocompromised patients. Amphotericin B is a polyene antifungal that binds irreversibly to ergosterol in fungal cell membranes, causing membrane disruption and cell death. Polymyxin E (colistin) is a cationic polypeptide antibiotic that disrupts the outer membrane of Gram-negative bacteria by binding to lipopolysaccharides and phospholipids, leading to increased permeability and bacterial cell death. Tobramycin is an aminoglycoside antibiotic that inhibits protein synthesis by binding to the 30S ribosomal subunit in susceptible bacteria, resulting in bactericidal activity against many Gram-negative organisms including Pseudomonas aeruginosa. This combination has been studied for prophylaxis against nosocomial pneumonia in ventilated patients and for reducing oral mucositis severity during chemotherapy[1][6]. The drugs act synergistically against a broad spectrum of pathogens—bacterial (Gram-negative) and fungal—and are typically administered topically rather than systemically due to toxicity concerns.
