Amulirafusp alfa (IMM0306) is a recombinant fusion protein that combines a monoclonal antibody against CD20 with the CD47 binding domain of signal-regulatory protein alpha (SIRPα) on both heavy chains. This bispecific biologic is designed to target both CD20 and CD47 on B cells. By simultaneously binding to these targets, it blocks the "don't eat me" signal mediated by the interaction between CD47 and SIRPα—thereby promoting phagocytosis by macrophages—and also engages natural killer (NK) cells through Fc receptor interactions. The drug exerts its anti-tumor effects via multiple mechanisms including antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP), and complement-dependent cytotoxicity (CDC). Amulirafusp alfa has demonstrated robust preclinical activity as well as promising early clinical efficacy in relapsed or refractory B-cell non-Hodgkin lymphoma subtypes such as follicular lymphoma, marginal zone lymphoma, and diffuse large B-cell lymphoma. It is being developed primarily by ImmuneOnco Biopharmaceuticals.
