This is a combination therapy consisting of anakinra, dexamethasone, and immunoglobulin (typically intravenous immunoglobulin, IVIG). - **Anakinra** is a recombinant human interleukin-1 receptor antagonist that blocks the activity of interleukin-1 alpha and beta, key cytokines in inflammatory processes. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive properties. - **Immunoglobulin (IVIG)** consists of pooled antibodies from healthy donors and modulates immune responses through multiple mechanisms including neutralization of autoantibodies and modulation of Fc receptors. This combination has been used off-label for the treatment of severe inflammatory conditions such as secondary hemophagocytic lymphohistiocytosis (HLH), macrophage activation syndrome (MAS), cytokine release syndrome (CRS), and other hyperinflammatory states. The rationale for combining these agents lies in their complementary mechanisms to suppress excessive immune activation—anakinra targets IL-1 signaling; dexamethasone broadly suppresses inflammation; IVIG provides passive immunity and further immune modulation[6][7][10].
