ANGPTL3 monoclonal antibodies are a class of biologic therapies designed to inhibit angiopoietin-like protein 3 (ANGPTL3), a protein primarily secreted by the liver that regulates lipid metabolism. ANGPTL3 acts as an inhibitor of lipoprotein lipase (LPL) and endothelial lipase (EL), enzymes responsible for the clearance of triglycerides and phospholipids from the blood. By neutralizing ANGPTL3, these monoclonal antibodies promote the activity of LPL and EL, resulting in significant reductions in low-density lipoprotein cholesterol (LDL-C), triglycerides, and non-high-density lipoprotein cholesterol. This mechanism is distinct from statins and PCSK9 inhibitors as it does not rely on the LDL receptor, making it particularly valuable for patients with homozygous familial hypercholesterolemia (HoFH). Key agents in this class include the FDA-approved evinacumab (Evkeeza) and the investigational antibody SHR-1918.
