Overview

anti-BMP9 refers to a class of monoclonal antibodies designed to selectively neutralize Bone Morphogenetic Protein 9 (BMP9), also known as Growth Differentiation Factor 2 (GDF2). The most prominent therapeutic candidate in this class is PF-07209326, developed by Pfizer in collaboration with researchers at Massachusetts General Hospital. BMP9 is a circulating ligand that binds with high affinity to the ALK1 receptor. While loss-of-function mutations in the GDF2 gene are associated with heritable pulmonary arterial hypertension (PAH), experimental evidence suggests that BMP9 can also drive pathological pulmonary vascular remodeling in established disease. It promotes the expression of vasoactive and pro-remodeling mediators, such as endothelin-1 (ET-1) and CXCL12, in pulmonary microvascular endothelial cells via non-canonical ALK1-SMAD3 signaling. Selective inhibition of BMP9 aims to attenuate vascular remodeling and improve hemodynamics in PAH patients. This approach is designed to be more selective than broad ligand traps (like ALK1-Fc) that inhibit both BMP9 and BMP10, thereby potentially avoiding adverse effects such as peripheral edema associated with BMP10 inhibition.
