Anti-IL-6 neutralizing antibodies are a class of monoclonal antibodies that bind directly to the interleukin-6 (IL-6) cytokine, thereby preventing it from interacting with its receptors (IL-6R and gp130). This mechanism of action inhibits both classic signaling and trans-signaling pathways, which are critical drivers of systemic inflammation and tissue remodeling. In the context of pulmonary arterial hypertension (PAH), IL-6 neutralization has been shown to reduce pulmonary vascular remodeling and improve right ventricular function by modulating the IL-6/STAT3/Foxp3 axis. Unlike IL-6 receptor antagonists, which target the receptor, these neutralizing antibodies target the ligand itself, potentially offering a more precise disruption of IL-6-driven signaling.
