Overview

Antithymocyte globulin (equine) is a horse-derived **polyclonal anti-human T-lymphocyte immunoglobulin** biologic immunosuppressant marketed as **Atgam**. It is a purified gamma globulin preparation generated from the hyperimmune serum of horses immunized with human thymus lymphocytes and is administered by **intravenous infusion**. The product depletes and functionally suppresses circulating T cells and other lymphocyte populations, thereby reducing cell-mediated immune injury. It is approved for treatment of **acute renal allograft rejection** and **moderate to severe aplastic anemia** in patients who are not suitable candidates for bone marrow transplantation. Historically associated with Pharmacia & Upjohn and currently within Pfizer's portfolio, it has also been used in clinical practice and publications for immunosuppressive conditioning or treatment settings such as **myelodysplastic syndrome** and **graft-versus-host disease**, although those uses are not core labeled indications.
