Overview

This combination consists of **atezolizumab** (a monoclonal antibody targeting PD-L1), **obinutuzumab** (a type II anti-CD20 monoclonal antibody), and **bendamustine** (an alkylating agent with antineoplastic activity). Atezolizumab blocks the PD-L1/PD-1 pathway to enhance T-cell-mediated anti-tumor immunity. Obinutuzumab promotes B-cell lysis via antibody-dependent cellular cytotoxicity and direct cell death mechanisms. Bendamustine induces DNA cross-linking leading to apoptosis. The regimen has been clinically evaluated mainly for **previously untreated follicular lymphoma**. In a phase Ib/II trial (NCT02596971), the combination showed high overall response rates but substantial toxicity, with frequent grade 3/4 adverse events. Though the regimen achieved a complete response rate of 75% in this population, the safety profile was unfavorable compared to standard obinutuzumab-based chemoimmunotherapy, and thus it cannot be recommended as standard of care for untreated follicular lymphoma[1][5][7].
