Autoantibody Reductive Therapy is an experimental combination treatment designed to reduce pathogenic autoantibodies in patients with acute exacerbations of idiopathic pulmonary fibrosis (AE-IPF). The regimen consists of therapeutic plasma exchange (TPE), rituximab, and intravenous immunoglobulin (IVIG). TPE physically removes circulating autoantibodies from the blood; rituximab is a monoclonal antibody that depletes B cells responsible for producing new autoantibodies; IVIG provides pooled immunoglobulins that can modulate immune responses and further neutralize pathogenic antibodies. This approach targets the underlying autoimmune component believed to contribute to lung injury in AE-IPF. Early pilot studies and ongoing clinical trials suggest this combination may improve oxygenation, ambulation, and survival in some patients who are otherwise refractory to standard therapies[1][4][5][6][8].
