Overview

This is a combination of four antibiotics—azithromycin, doxycycline, imipenem, and cilastatin. Each component has a distinct mechanism of action:

  • Azithromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit.
  • Doxycycline is a tetracycline antibiotic that also inhibits protein synthesis but binds to the 30S ribosomal subunit.
  • Imipenem is a carbapenem beta-lactam antibiotic with broad-spectrum activity against Gram-positive and Gram-negative bacteria; it inhibits cell wall synthesis by binding to penicillin-binding proteins.
  • Cilastatin is not an antibiotic but an inhibitor of renal dehydropeptidase I; it prevents the degradation of imipenem in the kidneys, thereby increasing its efficacy.

This combination would provide extremely broad antibacterial coverage—including many multidrug-resistant organisms—and could be considered for severe or mixed infections where multiple pathogens are suspected. Imipenem-cilastatin combinations are approved for use in serious infections such as sepsis, complicated intra-abdominal infections, urinary tract infections, and pneumonia. Azithromycin and doxycycline are often used for respiratory tract infections and atypical pathogens. There are reports on combining imipenem-cilastatin with azithromycin for community-acquired pneumonia, but no standard product or clinical guideline supports routine use of all four agents together.
