**Baricitinib + ciclosporin** is a combination of two immunosuppressive drugs, both small molecules, used experimentally to control immune-mediated diseases and prevent graft rejection. Baricitinib is a selective and reversible inhibitor of Janus kinase 1 and 2 (JAK1/2), modulating the signaling of cytokines involved in inflammatory and immune responses. Ciclosporin acts primarily by inhibiting calcineurin, blocking T-cell activation through suppression of interleukin-2 (IL-2) transcription. Preclinical models indicate this combination offers potent immunosuppressive effects, particularly in allograft survival, by affecting T cell subsets and inflammatory cell profiles. However, the combination is not generally recommended clinically due to a heightened risk for severe infection and lymphoproliferative complications, and pharmacokinetic interactions can increase baricitinib exposure owing to ciclosporin's inhibition of P-glycoprotein-mediated drug transport[1][4][9].
