Overview

This is a hypothetical combination of four Janus kinase (JAK) inhibitors—baricitinib, filgotinib, tofacitinib, and upadacitinib. Each is an oral small molecule that inhibits one or more members of the JAK family (JAK1, JAK2, JAK3, TYK2), which are cytoplasmic tyrosine kinases involved in cytokine signaling pathways central to immune and inflammatory responses. These drugs are individually approved for the treatment of autoimmune diseases such as rheumatoid arthritis and ulcerative colitis. They work by blocking pro-inflammatory cytokine signaling through inhibition of specific JAK isoforms; however, their selectivity profiles differ: - Baricitinib primarily inhibits JAK1 and JAK2. - Filgotinib is highly selective for JAK1. - Tofacitinib preferentially inhibits JAK1 and JAK3 but also affects other isoforms at higher concentrations. - Upadacitinib is a selective inhibitor of JAK1. There are no known clinical products or studies combining all four agents together as a single therapy; this entry describes the theoretical combination based on their individual pharmacology[5][6][8].
