**Baricitinib + topical corticosteroid** is a combination therapy used primarily for **moderate-to-severe atopic dermatitis (AD)** in adults who have an inadequate response to topical corticosteroid (TCS) therapy alone. Baricitinib is a selective, oral small molecule inhibitor of Janus kinase 1 (JAK1) and Janus kinase 2 (JAK2). By inhibiting the JAK-STAT signaling pathway, baricitinib reduces the production and signaling of pro-inflammatory cytokines central to AD and other autoimmune/inflammatory diseases. Topical corticosteroids are anti-inflammatory agents acting primarily via glucocorticoid receptor agonism, suppressing multiple components of the inflammatory response in the skin. The combination offers additive and complementary control over immune dysregulation and inflammation in AD. Clinical trials (such as BREEZE-AD7 and other phase 3 studies) have demonstrated that this combination therapy provides superior, sustained efficacy compared to TCS alone, improving symptoms for up to 52–68 weeks, though with increased mild-to-moderate adverse events.
