Overview

BCMA bispecific antibodies are a class of T-cell engaging immunotherapies designed to treat plasma cell malignancies, primarily multiple myeloma and light chain (AL) amyloidosis. These agents are engineered to simultaneously bind to the B-cell maturation antigen (BCMA) on the surface of plasma cells and the CD3 receptor on T-cells. This dual binding brings T-cells into close proximity with malignant cells, leading to T-cell activation and the release of cytotoxic proteins that induce apoptosis in the target cells. Several agents in this class, such as teclistamab, elranatamab, and linvoseltamab, have received regulatory approval for the treatment of relapsed or refractory multiple myeloma. They are also being investigated for use in earlier lines of therapy and for other BCMA-expressing disorders like AL amyloidosis, where they have shown high rates of hematologic complete response in clinical studies.
