Overview

BCMA-CART-STRIKE is an experimental chimeric antigen receptor (CAR) T-cell therapy developed by the Mayo Clinic for the treatment of multiple myeloma. The therapy consists of T cells engineered to target B-cell maturation antigen (BCMA) on myeloma cells while simultaneously secreting multi-specific molecules known as STriKEs (Secreted Tri-specific/Bi-specific molecules). These STriKEs are designed to remodel the immunosuppressive bone marrow tumor microenvironment by targeting fibroblast activation protein (FAP) on inhibitory cancer-associated fibroblasts (iCAFs). Two STriKE strategies have been described: a tri-specific molecule (incorporating IL-15, anti-FAP, and anti-CD16) to activate innate immune cells against CAFs, and a bi-specific Fc-fusion molecule (targeting anti-FAP and anti-SIRPα) to induce macrophage-mediated phagocytosis of CAFs. This dual-action approach aims to overcome CAF-mediated resistance and improve the durability of responses in multiple myeloma patients.
