Belatacept + belimumab is a combination of two biologic immunosuppressive agents used off-label in select cases, such as kidney transplantation in patients with concurrent systemic lupus erythematosus (SLE). Belatacept is a selective T-cell costimulation blocker that inhibits T lymphocyte activation by binding to CD80 and CD86 on antigen-presenting cells, thereby preventing their interaction with the CD28 receptor on T cells. Belimumab is a fully human monoclonal antibody that targets and neutralizes B-lymphocyte stimulator protein (BLyS/BAFF), reducing B cell survival and differentiation. This combination may be considered for complex autoimmune or transplant scenarios where both T cell and B cell modulation are desired[2][3][1].
