Overview

Beta-lactam + azithromycin is a combination antibiotic regimen frequently utilized for the treatment of hospitalized patients with community-acquired pneumonia (CAP). This combination therapy pairs a beta-lactam antibiotic (such as ceftriaxone, cefotaxime, or ampicillin-sulbactam) with azithromycin, a macrolide. The beta-lactam component acts by inhibiting penicillin-binding proteins (PBPs), thereby disrupting bacterial cell wall synthesis, while azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. This dual approach provides comprehensive coverage against both typical bacterial pathogens and atypical organisms like *Mycoplasma pneumoniae* and *Legionella* species. Clinical guidelines often recommend this combination as a standard-of-care alternative to fluoroquinolone monotherapy in the inpatient setting.
