Bosentan + pacritinib is a combination of two small molecule drugs, each with distinct mechanisms and indications. Bosentan is an endothelin receptor antagonist used primarily for the treatment of pulmonary arterial hypertension (PAH), where it blocks the action of endothelin-1 at both ETA and ETB receptors, leading to vasodilation and reduced blood pressure in pulmonary arteries[1][2][5]. Pacritinib is an oral kinase inhibitor indicated for adults with intermediate or high-risk primary or secondary myelofibrosis, especially those with low platelet counts. It selectively inhibits Janus kinase 2 (JAK2) as well as other kinases such as FLT3 and JAK3, thereby reducing abnormal cell proliferation associated with myeloproliferative neoplasms[3][4][6]. There are no approved indications for this specific drug combination; however, clinical studies have evaluated their pharmacokinetic interactions[7].
